New Patient Forms:

If you are a new patient, please fill out the patient registration and acknowledgment of privacy notice. This form can be downloaded by clicking the button below. Please print, complete all fields and bring with you to your appointment.

What do I need to bring to my appointment?

  • Lab or diagnostic imaging reports. If you don't have the report, please let us know where the lab or imaging test was performed and we can request the report for you.

  • Current medications: name of the medication, milligrams, and dosage.

  • Any supplements you are taking.

What can I do to prepare for my first visit?

Dr. Roberts’ office staff will help you request copies of your medical records and labs so they are available for review during your first visit. Fill out your New Patient Packet prior to arrival at the clinic so that Dr. Roberts can review your health concerns and medical history. Please also bring any supplements you are taking. If you are going to seek insurance reimbursement, please familiarize yourself with your out-of-network coverage.

Do you accept my insurance?

Early in Dr. Roberts’ practice, she realized she would not be able to deliver the highest quality of care that she is committed to delivering if she allowed insurance companies to dictate how she practiced medicine. She has since decided to opt out of the third party insurance payment model. This allows her to increase the amount of time spent with each patient, an integral component of her integrative and highly individualized medical model. This is part of Dr. Roberts’ commitment to providing solutions for all of your health challenges, no matter how chronic or complex they may be.

Payment in full is expected for services rendered on the day of your visit. We accept cash, checks, and major credit cards. Our office staff would be happy to make claim forms available to you, upon request, to submit to your insurance carrier for direct reimbursement, where applicable. Unfortunately we are not able to determine how much your particular insurance provider might reimburse you for services as there are so many different plans and they vary so much. We recommend that you call your specific insurance provider and ask if they have naturopathic (ND) coverage.

What does my insurance cover?

Labs, imaging and pharmaceutical prescriptions that Dr. Roberts may recommend are generally covered by most insurance plans. Coverage for office visits varies by particular insurance plan and carrier. Please familiarize yourself with your plan’s coverage policy.
